The Client-Directed Home Care (CDHCI) program in Alberta is designed to give individuals more independence in how their home care is delivered. Instead of following a fixed care model, eligible participants can take an active role in organizing and tailoring the support they receive at home to better match their personal needs and preferences.

With this authorization, we are able to provide flexible, personalized home care services throughout Alberta, empowering clients to make informed choices about their care and how it is delivered.

Client Independence and Decision-Making

The CDHCI program is built around giving clients greater authority over their home care. Individuals can select their preferred caregivers, decide when services are provided, and influence the type of support they receive.

Financial Flexibility

  • Assistance with daily living activities like bathing, dressing, and grooming

Self-Managed Care (SMC) Program

The Self-Managed Care (SMC) program provides eligible individuals with direct funding from Alberta Health Services (AHS), enabling them to hire and oversee their own caregivers. This model gives clients full control over how their care is organized and delivered.

Eligibility Requirements

To participate in the SMC program, individuals must:

  • Be assessed by Alberta Health Services as needing home care support
  • Have stable health conditions that can be safely managed at home
  • Be able to manage employer responsibilities, or have a trusted representative who can do so on their behalf

Use of Funds

Funding received through the program can be used for services such as:

  • Personal care (e.g., bathing, dressing, grooming)
  • Household assistance (e.g., cleaning, meal preparation)
  • Respite care to support family caregivers
  • Additional services outlined in the approved care plan

How the Program Works

  • Assessment: A Home Care Case Manager evaluates the client’s needs and determines eligibility
  • Funding Agreement: Approved clients receive a formal agreement outlining funding and usage
  • Care Management: The client or their representative takes responsibility for recruiting, training, and supervising caregivers
